This form is used by the Owner to make accounting requests of Olympia Management Services (OMS) for refunds of deposits, credit balances, late payments or collection fees as well as transfers from one account to another. Documentation must accompany this form.

This form is used to cancel the automatic withdrawal of payments from your account or to change the bank or account number used for the withdrawal of monies.

 

This form is used to submit for Architectural Review of any exterior improvements or change to the home, yard, or other property. It may be necessary to complete this form even if there are pre-existing improvements that were not reviewed by the Association at time of installation. All exterior changes, installations, and improvements require Architectural Review in accordance with the governing documents and the Southern Highlands Homeowner Design Manual. The Application is NOT the Design Manual.

This form is used by the Owner to authorize the automatic withdrawal of payments form one’s bank account on a recurring or one-time basis. Use of ACH is highly encouraged and reduces the possibility of late or missed payments.

This form is used by a Seller to request individual copies of the documents, such as the Certificate, which is required by NRS 116.4109 to be provided to a buyer at the time of sale. This form is used by the Owner to make alterations to the information contained in or on an account such as a change in mailing address or contact information, the addition or deletion of an authorized contact, the authorization of a property manager, or a change to the names on an account.

This form is used by an Owner with special circumstances to request a variance from enforcement of the community parking regulations. Generally, variances are only given for disability accessibility or emergency 24-hour service vehicles.

This package is used when reserving a park for private or corporate use.

This form is used by an Owner wishing to request a variance from the approved criteria as set forth in the Association’s Design Manual after an Architectural Review application has been submitted and denied. This form should not be used to request a parking variance.

This form is used by an Owner with outstanding fines who wishes to request a review of a compliance balance by the Board of Directors. The Board respectfully requests, when possible, that any outstanding violation be cured and closed prior to submitting a request.

A Neighborhood Association may complete a form for the rental of a Southern Highlands approved Community Yard Sale Banner.

This form is used by an Owner or Resident wishing to rent a Southern Highlands Community Association approved yard sale sign for individual use.

This form is used to request a Resale Packet. The Resale Packet includes the Certificate Documents defined in NRS 116.4109(3b) and other disclosure documents that are typically provided to a purchaser during the sale of a property. There is a service fee for the preparation and delivery of materials requested on this form.
